The institution was established in 1991 following a presidential announcement made during an official visit to Kwara State by the then Military President, Gen. Ibrahim Badamasi Babangida. The Polytechnic operates two campuses:

• Main Campus – Ojoku Road, Offa.
• Mini Campus – Osogbo Road, Offa, Kwara State.

• Clerical Officer – Bachelor’s Degree (Second Class Lower) in Medical Laboratory Science + IMLS registration + NYSC. (Grade: CONTEDISS 03)
• Senior Clerical Officer – SSCE with 4 credits in one sitting or 5 credits in two sittings. (Grade: CONTEDISS 04)
• Confidential Secretary III – ND in Office Technology & Management/Secretarial Studies. (Grade: CONTEDISS 06)
• Confidential Secretary I – HND in Office Technology & Management or equivalent + NYSC. (Grade: CONTEDISS 08)

Medical & Health Roles

• Senior Medical Officer – MBBS + 3 years post-qualification experience + MDCN registration. (Grade: CONMESS 03)
• Medical Officer – MBBS + MDCN registration. (Grade: CONMESS 02)
• Higher Environmental Health Assistant – National Certificate of the Nigerian Health Records Association / HRORBN. (Grade: CONHESS 04)
• Staff Nurse / Midwife II – NRN or NRM Certificate. (Grade: CONHESS 07)
• Staff Nurse / Midwife I – NRN / NRM Certificate + 3 years post-qualification experience. (Grade: CONHESS 08)
• Pharmacist – B.Pharm (minimum of Second Class Lower) + PRBN registration + NYSC. (Grade: CONHESS 08)
• Medical Laboratory Scientist – B.Sc (Second Class Lower) in Medical Laboratory Science + IMLS registration + NYSC. (Grade: CONHESS 08)

Academic Roles

• Lecturer I – Master’s + 6 years teaching/research experience with publications OR PhD + 3 years teaching/research experience with publications. (Grade: CONPCASS 06)
• Lecturer II – Master’s + 6 years experience with journal/conference papers. (Grade: CONPCASS 05)
• Lecturer III – Bachelor’s Degree + 3 years teaching/research with publications. (Grade: CONPCASS 03)
• Assistant Lecturer – Bachelor’s Degree (Second Class Lower) in relevant discipline + NYSC. (Grade: CONPCASS 02)
• Senior Lecturer – Master’s + 12 years teaching/research with multiple publications OR PhD + 9 years relevant experience. (Grade: CONPCASS 07)
• Higher Instructor – HND (Credit level) + NYSC + TTC. (Grade: CONPCASS 02)
